Pharmacist Assessment Report

Procarbazine: From Hodgkin Lymphoma to Follicular Lymphoma

One-Sentence Summary

Procarbazine is an oral alkylating-type chemotherapy agent, originally used to treat Hodgkin lymphoma. The TxGNN model predicts it may be effective for follicular lymphoma, with 2 related clinical trials (neither directly testing procarbazine) and 12 publications currently supporting this direction, mostly older reports and small case series.

Quick Overview

Item Content
Original Indication Hodgkin lymphoma (including classic Hodgkin lymphoma and related subtypes)
Predicted New Indication Follicular lymphoma
TxGNN Prediction Score 0.9946
Evidence Level L3
Canada Market Status Marketed (marketed DINs: 1 of 1)

Recommended Decision Hold

Why is This Prediction Reasonable?

Currently, detailed mechanism of action data is not available. Based on known information, procarbazine is an oral alkylating-type agent with activity against lymphoma. Its efficacy in Hodgkin lymphoma is established, and mechanistically it may be applicable to follicular lymphoma.

Hodgkin lymphoma and follicular lymphoma are both lymphoid malignancies, and procarbazine’s cytotoxic activity against lymphoid cells provides biological plausibility. Procarbazine has also been used historically in non-Hodgkin lymphoma. One report describes two patients with relapsed or refractory follicular lymphoma who achieved a complete and durable remission with a prolonged course of daily procarbazine (PMID 16690522). A low-dose oral metronomic regimen containing procarbazine (PEP-C) has also been tested in relapsed or refractory lymphoma.

There is an important caveat. Current follicular lymphoma standards are anti-CD20 based, so procarbazine’s role would be at most a niche salvage or combination component.

Clinical Trial Evidence

Trial Number Phase Status Enrollment Key Findings
NCT00003113 Phase 2 Terminated 6 Oral combination chemotherapy with G-CSF in elderly patients with intermediate- or high-grade non-Hodgkin lymphoma. Terminated after only 6 participants, so it gives almost no usable efficacy evidence. The regimen composition is not confirmed.
NCT00577993 Phase 3 Completed 210 Fludarabine, mitoxantrone and dexamethasone plus rituximab in stage IV indolent lymphoma. The regimen does not include procarbazine, so it serves only as disease-context information.

Cytotoxicity

Item Content
Cytotoxicity Classification Conventional cytotoxic (alkylating-type agent)
Myelosuppression Risk Please refer to the package insert warnings and precautions. The knowledge graph links procarbazine with contraindications such as thrombocytopenia, leukopenia and anemia, which points to haematological toxicity.
Emetogenicity Classification Please refer to the package insert warnings and precautions
Monitoring Items Complete blood count with platelets, liver and renal function
Handling Protection Must follow cytotoxic drug handling regulations

Safety Considerations

  • Contraindications (not label text): liver failure, liver disease, kidney disease, gallbladder disease, thrombocytopenia, leukopenia, anemia (including macrocytic anemia), blood coagulation disease, peripheral neuropathy and other peripheral nervous system disease, infectious disease (including fungal infection and dermatophytosis), pyoureter, and substance abuse or dependence.
  • Drug Interactions: 100 interactions were identified in total. The main ones are classed as Major, and include:
    • Sympathomimetics and stimulants/anorectics: isometheptene, ephedrine, phentermine, diethylpropion, mazindol
    • Serotonergic agents: bupropion, lorcaserin, fenfluramine, dexfenfluramine
    • Opioid: morphine
    • 5-HT3 antagonists: ondansetron, granisetron, palonosetron, dolasetron

Moderate interactions include epinephrine (in several forms), ephedrine (nasal) and metoclopramide. Levofloxacin is Minor.

  • Other signals from the literature: procarbazine-containing regimens are associated with gonadotoxicity (such as azoospermia and ovarian failure), and animal bioassays show carcinogenicity.

Official warnings and contraindications from the Canadian package insert were not available and should be reviewed.

Conclusion and Next Steps

Decision: Hold

Rationale: The evidence consists of small case reports, older chemotherapy-combination studies and one terminated Phase 2 trial with only 6 participants. No trial directly tests procarbazine in follicular lymphoma. Modern anti-CD20-based therapy is the standard of care, and procarbazine carries notable toxicity (myelosuppression, gonadotoxicity, secondary malignancy, and many serious drug interactions). The prediction is best treated as a research question for now.

To proceed, the following is needed:

  • Health Canada package insert warnings and contraindications (a blocking gap for safety screening)
  • Mechanism of action data
  • Identification of the regimen and histology in NCT00003113, and a review of PEP-C outcomes in follicular lymphoma
  • Comparison against current anti-CD20-based standards to define any niche salvage role
  • A safety-monitoring and drug-interaction management plan
