Pharmacist Assessment Report

Selumetinib: Familial generalized lentiginosis

One-Sentence Summary

Selumetinib is a MEK1/2 inhibitor originally used for neurofibromatosis. The TxGNN model predicts it may be effective for peripheral nerve schwannoma, the best-supported of its top 10 predictions. The evidence is early-stage: 3 clinical trials (one terminated, two not yet recruiting) and 6 publications, mostly reviews, preclinical work and case reports.

Quick Overview

Item Content
Original Indication Not recorded in the local data
Predicted New Indication Familial generalized lentiginosis
TxGNN Prediction Score 0.9996
Evidence Level L5 (as assigned in the compiled data; note that no completed Phase 2/3 RCT exists, so this is generous)
Canada Market Status Marketed (marketed DINs: 2 of 2)
Number of DINs 2 (marketed: 2)
Recommended Decision Hold (pending further evidence; see Conclusion)

Why is This Prediction Reasonable?

Detailed mechanism-of-action data are not currently available in the compiled data. Based on known pharmacology, selumetinib is a MEK1/2 inhibitor that blocks the RAS/RAF/MEK/ERK signalling pathway.

In neurofibromatosis, loss of NF1 or NF2 leads to hyperactivation of this pathway in Schwann cells, driving nerve sheath tumour growth. Selumetinib is already approved for NF1 plexiform neurofibromas, a closely related nerve sheath tumour. Schwannomas arise from the same cell lineage and share this signalling dependence, particularly in NF2-related schwannomatosis (NF2-SWN). This gives a plausible mechanistic basis for the prediction.

Reviews and preclinical work in the compiled literature are consistent with this idea. However, efficacy in schwannoma itself has not been demonstrated in a completed study.

Clinical Trial Evidence

Trial Number Phase Status Enrollment Key Findings
NCT03095248 Phase 2 Terminated 10 Selumetinib in NF2-related tumours, assessing tumour growth and hearing changes. Directly relevant design, but terminated early with very limited data
NCT07707947 Phase 2 Not yet recruiting 20 Single-centre, single-arm study of selumetinib in adults and children with inoperable or progressive NF2-SWN. No results yet
NCT07713745 Phase 2 Not yet recruiting 200 PRIME-NF2 adaptive platform-basket trial of novel agents in NF2-SWN (vestibular and non-vestibular schwannomas, meningiomas, ependymomas). Unclear whether selumetinib is included

    • Cobicistat

Moderate-level interactions include several antithrombotic and antiplatelet agents (e.g., acetylsalicylic acid, apixaban, abciximab, bivalirudin, cilostazol, dipyridamole), as well as cimetidine, clotrimazole and deferasirox.

Package insert warnings and contraindications were not available; please refer to the Health Canada package insert for these.

Conclusion and Next Steps

Decision: Hold

Rationale: The mechanistic rationale is strong, and selumetinib is already marketed in Canada for a related nerve sheath tumour. However, evidence in schwannoma consists of one terminated 10-patient Phase 2 trial, two trials not yet recruiting, and a case report in ependymoma. No completed efficacy dataset exists. The other nine top predictions have little or no supporting evidence. Rhabdoid tumour has only indirect preclinical and umbrella-trial support.

To proceed, the following is needed:

  • Results or data from the terminated NCT03095248 trial
  • Confirmation of whether selumetinib is included in PRIME-NF2, and follow-up on NCT07707947 once it recruits
  • Package insert warnings and contraindications from Health Canada (needed for safety screening)
  • Mechanism-of-action data from DrugBank
  • A review of Major drug interactions (e.g., strong CYP3A4 inhibitors) for the target population
