Pharmacist Assessment Report

Topotecan: From Ovarian Cancer to Female Breast Carcinoma

One-Sentence Summary

Topotecan is a chemotherapy drug originally used to treat ovarian cancer. The TxGNN model predicts it may be effective for female breast carcinoma, with 3 registered clinical trials and 20 publications currently supporting this direction. Most of these are early-phase, terminated or indirect, so the evidence is limited.

Quick Overview

Item Content
Original Indication Ovarian cancer (including several ovarian subtypes such as germ cell tumour and yolk sac tumour)
Predicted New Indication Female breast carcinoma
TxGNN Prediction Score 0.9992
Evidence Level L5
Canada Market Status Marketed (marketed DINs: 4 of 4)
Number of DINs 4 (marketed: 4)
Recommended Decision Hold

Why is This Prediction Reasonable?

Detailed mechanism of action data is not available in the data compiled for this report. Based on known pharmacology, topotecan is a topoisomerase I inhibitor. It causes DNA damage in rapidly dividing cells, and its efficacy in ovarian cancer has been established.

Preclinical work suggests that topoisomerase I inhibition can produce synthetic lethality in MYC-driven cancers through abnormal accumulation of R-loops. This is relevant to MYC-amplified breast cancer subtypes such as triple-negative disease. Topotecan is also a substrate of the BCRP/ABCG2 efflux transporter, which is expressed in breast cancer and may drive drug resistance.

The very high TxGNN score supports biological plausibility, but it is a model prediction and not clinical evidence.

Clinical Trial Evidence

Trial Number Phase Status Enrollment Key Findings
NCT00006032 Phase 2 Terminated Not reported Intensive-dose topotecan, ifosfamide/mesna and etoposide followed by autologous stem cell rescue in metastatic breast cancer; the multi-agent, high-dose regimen means topotecan’s own contribution cannot be isolated
NCT02419495 Phase 1 Terminated 221 Safety of selinexor combined with several standard chemotherapies in advanced solid tumours; no breast-specific topotecan signal
NCT06840886 Phase 1 Recruiting 426 First-in-human dose escalation and expansion study of PHST001 in advanced solid tumours; topotecan’s role is unclear and no efficacy results exist

Cytotoxicity

Item Content
Cytotoxicity Classification Conventional cytotoxic (topoisomerase I inhibitor, camptothecin class)
Myelosuppression Risk High (neutropenia, thrombocytopenia and anaemia are consistent with the contraindication-related conditions listed in the knowledge graph)
Emetogenicity Classification Please refer to the package insert warnings and precautions
Monitoring Items Complete blood count with differential, liver and renal function
Handling Protection Must follow cytotoxic drug handling regulations

Safety Considerations

  • Contraindication-related conditions (from the knowledge graph, not label text): thrombocytopenia, neutropenia, agranulocytosis, anaemia, macrocytic anaemia, interstitial lung disease, diarrhoeal disease, impaired renal function, pulmonary fibrosis, postinflammatory pulmonary fibrosis.
  • Drug interactions: 100 interactions were recorded. The more significant ones include:
    • Major: deferiprone, samarium (153Sm) lexidronam
    • Moderate: eliglustat, rolapitant, iobenguane (I-131), palifermin, strontium chloride Sr-89, ibritumomab tiuxetan
    • Minor: levofloxacin, caffeine

Official package insert warnings and contraindications were not available in the data compiled for this report. Please refer to the package insert for complete safety information.

Conclusion and Next Steps

Decision: Hold

Rationale: The prediction score is very high, but the supporting clinical evidence for breast cancer is weak: the only breast-specific Phase 2 study was terminated, one infusional Phase 2 study showed no increased efficacy, and the rest is preclinical, review or indirect material. Package insert safety data are also missing, so safety screening cannot proceed yet.

To proceed, the following is needed:

  • Health Canada package insert warnings and contraindications (currently a blocking gap)
  • Mechanism of action data from DrugBank
  • Clinical data specific to topotecan in breast cancer, particularly triple-negative or MYC-driven subtypes
  • A review of the myelosuppression, pulmonary and renal risks listed above before any further study
